Magnetverankerte prothetischeVersorgung nach Onlayosteoplastik und simultaner Implantation im atrophierten Oberkiefer

Zusammenfassung:
Die Therapie des extrem atrophierten Oberkiefers durch Onlayosteoplastik und simultane Implantat-insertion ist mit hohen initialen Implantatverlustraten und einem relativ ausgeprägten vertikalen Knochenabbau in den ersten beiden Jahren behaftet. Die vorliegende Studie sollte klären, ob sich die Implantatverlustrate und Knochenatrophie durch eine modifizierte prothetische Versorgung mit magnetverankerten Totalprothesen verringern lassen. Es zeigte sich allerdings, daß die erwarteten biomechanischen, technischen und werkstoffkundlichen Vorteile nur teilweise eintrafen. Eine Implantatverlustrate von 25% und ein durchschnittlicher Knochenabbau von 2,9 mm ein Jahr nach prothetischer Versorgung und 18 Monate nach der Onlayosteoplastik mit simultaner Implantatinsertion bedeuten keine Verbesserung hinsichtlich dieser beiden Parameter im Vergleich mit rein implantatgetragenen, bedingt abnehmbaren und stegretinierten protheti-schen Suprakonstruktionen. Indiziert ist die Versorgung mit einer magnetverankerten Prothese nach Onlayosteoplastik und Implantation jedoch bei ungünstigen anatomischen Verhältnissen und bei Patienten, die in ihrer manuellen Geschicklichkeit eingeschränkt sind.
Summary:
The reconstruction of severely resorbed edentulous maxillae with onlay bone grafts and implants placed in the same session is associated with high initial implant failure rates and rather pronounced marginal bone loss during the first 2 years. This study was designed to answer the question, if a modified prosthodontic treatment approach using magnet-retained cover dentures might reduce implant failures and ridge resorption. However, these restorations failed to show the expected favorable biomechanical, technical and material properties. An implant loss rate of 25% and an average bone loss of 2.9 mm per year following denture insertion and 18 months after onlay grafting plus simultaneous implant placement is no improvement of these two parameters compared with purely implant-supported fixed-removable bar-retained restorations. Yet, magnet-retained dentures after onlay grafting and implant placement do have their indications for unfavorable anatomic situations and patients with limited manual skills.
